Do I need to make an appointment?

Yes, you will need to call 330.812.3816 or use our online form to schedule an appointment and find out what documentation you will need to bring to your appointment.

Who may become a patient at the dental clinic?

Our dentist will see people of all ages.

I am having dental pain? Will you see me at your clinic?

If you have swelling or pain, please call 330.812.3816. We may be able to see you the same day.

Do you accept private insurance?

Yes. The clinic accepts Medicaid and most Medicaid HMOs, including Buckeye, United Healthcare, Care Source, Molina and Paramount. We also accept Guardian and Delta Dental as well as a few other private dental insurances. Please call your insurance carrier to check that we are a provider for your insurance. 

Is this a free clinic?

We are not a free dental clinic. We offer a sliding fee discount program (based on income) for patients without insurance or who have limited insurance. We can assist with Medicaid applications for those who appear to be eligible. Payment plans are available for dentures and partial dentures.

How does the sliding fee scale work?

To qualify for the sliding fee discount program, you will be required to fill out an application and provide proof of income for all family members living in you household. Our scheduler and/or billing specialist will tell you what paperwork you will need to provide to qualify for a reduced fee for services. These may include:

  • one month of the most recent pay stubs
  • bank deposits
  • social security determination letter
  • prior year tax return
  • W-2 
  • completed sustainability form

I have Medicare coverage and need to see the dentist. What are my options?

Medicare does not cover dental services. However, if you are on disability, have Medicare Parts A, B, and D and are over the age of 18 years, you may qualify for additional benefits through the +MyCare Ohio Plan.
