Report a Facility Outbreak

A general definition of an outbreak is defined as two or more cases of similar illness with a common exposure.

Any outbreak or significant increase in infectious diseases above the norm or baseline must be reported to the local health district, since the outbreaks are considered reportable as a “Class C” disease as outlined in the Ohio Administrative Code (OAC) Rule 3701-3-02.  

Outbreaks must be reported by the end of the next business day in accordance with this rule.

This submission portal is meant for facilities (Long Term Care, Daycares, Schools etc.) reporting a confirmed or suspected outbreak.

 

This submission portal is not to report restaurant complaints.  Instead go to Complaints | Summit County Public Health.

 

In addition to reporting to Summit County Public Health, Long Term Care facilities need to report the outbreak to the Long Term Care Bureau at 330-643-1300.
