Caring for a loved one with a chronic illness, memory loss, or age-related challenges is a profound act of love—but it can also be deeply overwhelming. The system is complicated, and it is easy to feel lost.
You don't have to navigate it alone. We provide the direction.
The Care Compass is a completely free, telephone-based support program that pairs you with a dedicated Care Navigator to help your family chart a clear path forward.
How your Care Navigator will help you:
- Chart Your Options: We help you find and connect with local community resources (like meal delivery, transportation, and home care) that you might not know exist.
- Build a Care Map: We work collaboratively with you to create a simple, manageable step-by-step plan for daily care and safety at home.
- Provide Steady Guidance: Through regular phone check-ins, we offer ongoing emotional support and adjust your care plan as health needs change over time.
Who is eligible? This grant-funded program is entirely FREE and currently available to residents living in Portage, Stark, Summit, and Wayne counties.
