Ordering Ohio Death Certificates Made Quick and Easy for Funeral Directors
Summit County Public Health issues registered death certificate copies for every county in the State of Ohio to all funeral directors even if the death occurred in another Ohio county.
Summit County Public Health accepts:

- Call Ahead and Same Day death certificate ordering
- Dedicated funeral home customer service counter
- Updated Supplemental Death Certificate and Death Correction Affidavit replacement at no additional charge, Return of previously purchased death certificates is not required (*Terms and restrictions apply)
- Guaranteed mail order delivery as soon as the next business day
Emailed and faxed death certificate ordering:
Complete an Application for Certified Copies form and email it to vitalscph@schd.org. Listing multiple names on one application is acceptable.
Once the order has been prepared, an “Order Ready for Pickup” email from SCPH will be sent to the funeral home. Funeral homes should wait to receive the “Order Ready for Pickup” email before sending funeral home personnel to SCPH.
Standardized fee and payment options for all Ohio funeral homes:
The fee for certified death certificate copies issued by SCPH is $22.00 per copy for all Ohio funeral homes even if the death occurred in another Ohio county. SCPH accepts cash, check, money orders, cashier’s checks, Visa, MasterCard, American Express, and Discover Card. Funeral homes that will pay by check and plan to pick up orders the same business day may provide the check number and check amount when ordering to allow the immediate completion of the death certificate order. The actual check must be presented at the time of pickup to receive the processed order.
Call Ahead and Same Day death certificate ordering:
Call 330-812-3846 to request the number of death certificates needed by telephone. Provide the method of payment and specify if the order will be for same day pick up. Payments by check must present the actual check when picking up the ordered death certificates.
Dedicated funeral home customer service counter:
SCPH has customer service counters dedicated to servicing funeral home customers separate from those serving community customers. This two customer counter system ensures that SCPH Birth and Death Record customers receive seamless and efficient quality customer service. Lobby seating is also available for in person ordering.
Updated Supplemental Death Certificate and Death Correction Affidavit replacement at no additional charge:
SCPH will replace updated supplemental and death affidavit corrected death certificate copies based on the quantity of records originally purchased from our local health department a maximum of two times at no additional charge. Thereafter, the fee amount of $22.00 will be charged for each supplemental and death affidavit issued.
Guaranteed mail order delivery as soon as the next business day:
SCPH provides guaranteed mail order delivery for all orders that paid for USPS and UPS shipping as soon as the next business day. Standard USPS First-Class regular mail order delivery is not guaranteed and if the order is lost, stolen, delayed or undelivered, SCPH will not replace the order. USPS First-Class regular mail order delivery that is damaged during transit will be replaced after sufficient proof is provided.
