The Gatekeeper Program: Community Eyes & Ears for Seniors in Need

(Watching Out for Older Adults & Caregivers)

Not everyone has a family member nearby to check in on them. Many older adults living with memory loss, chronic illness, or severe isolation struggle behind closed doors and may be hesitant or afraid to ask for help.

You can be the bridge to safety.
The Gatekeeper Program trains everyday community members, local business workers, and first responders to recognize the subtle warning signs that an older adult or caregiver needs assistance—connecting them to vital support before a crisis occurs.

Signs an Older Adult or Caregiver May Need Help
Gatekeepers are trained to notice early red flags in daily life, such as:

  • Memory & Confusion: Forgetfulness, getting lost in familiar places, or unexplained personality changes.
  • Living Conditions: A neglected yard or pet, accumulated clutter, or a home in need of urgent repairs.
  • Personal Care: An unkempt appearance, dirty clothing, or noticeable body/home odors.
  • Social & Emotional: Living completely isolated, difficulty managing finances/bills, or visible caregiver exhaustion.

How the Program Works
When a Gatekeeper submits a referral, a Summit County Public Health social worker reviews the situation and conducts a respectful, compassionate home visit. We build trust, assess their overall needs, and create a personalized plan to help the senior remain safe, healthy, and independent in their own home.
