Home Choice

When youth have extremely intense behavioral health issues, they sometimes need short term care at a residential treatment facility for stabilization.  In these cases, the goal is always reunification with family and/or community, and services being provided in the least restrictive manner.  FCFC HOME Choice Transition Coordination services help successfully transition youth from residential care back into the family home and/or community setting.

To qualify, youth must be Medicaid eligible, placed in a 16+ bed treatment center for at least 90 days, and relocate to a residence with no more than 4 unrelated individuals in the residence.     

Funding for the HOME Choice program is provided by Ohio Dept. of Medicaid and the federal Money Follows the Person (MFP) Program.
