Care Compass

Caring for an aging or ill loved one is a profound act of love, but you shouldn't have to navigate the overwhelming maze of caregiving alone. The Care Compass(a BRI Care Consultation™ Program) is a completely free, telephone/email-based support service for family caregivers in Portage, Stark, Summit, and Wayne counties. We pair you with a dedicated Care Navigator who provides steady emotional support, connects you to local community resources you might not know exist, and works alongside you to build a manageable, step-by-step plan for daily care. :Learn more about Care Compass and how to get started.


Gatekeeper Program

Not everyone has family nearby to check in on them, and many vulnerable seniors living with memory loss or isolation struggle in silence. The Gatekeeper Programtrains everyday community members, local workers, and first responders to be the "eyes and ears" of our neighborhoods. By learning to spot subtle warning signs—like personal neglect, home deterioration, or severe caregiver stress—Gatekeepers can make a quick referral to Summit County Public Health. Our team of social workers then step in with compassionate home visits and tailored support to help older adults stay safe and independent. Learn more about the Gatekeeper Program.


BRIDGES: Transitions of Care

Coming home from the hospital brings a wave of new medications, appointments, and challenges, but you do not have to manage the transition alone. The BRIDGES: Transitions of Care Programhelps you improve your quality of life by supporting your recovery safely at home. We connect you with a social worker who ensures you feel confident managing your care, helps you avoid unnecessary hospital visits, and links you to vital local support so you can stay safe and independent. We currently partner with Summa Health systems for direct patient referrals—if you are at another local hospital, ask your care team about connecting with Summit County Public Health!
