BRIDGES: Transitions of Care Program

(Connecting Care. Supporting Recovery.)

Coming home from an acute care hospital can be an overwhelming experience. Between navigating new medications, scheduling follow-up appointments, and adjusting to your routine, it is easy to feel lost or anxious. The BRIDGES program is designed to help you improve your quality of life by supporting your recovery safely at home.

How We Support Your Recovery
Our dedicated social workers step in during the transition from the hospital to home, providing critical guidance to minimize potential setbacks. We work closely with you and your family caregivers to ensure you:

  • Feel Confident: We help you understand your discharge instructions and feel capable of managing your ongoing care.
  • Stay Home: We help troubleshoot barriers (like securing transportation or obtaining medical equipment) to avoid unnecessary hospital readmissions.
  • Stay Independent: We connect you with local community support and resources to ensure your long-term safety.

Hospital Partnerships & Referrals
We currently partner directly with Summa Health systems to receive patient referrals. If you or a loved one are discharging from a Summa hospital, your care team can submit a referral directly to our social workers.

Are you receiving care at a different local hospital? Please let your hospital's discharge planners know you are interested in partnering with Summit County Public Health's BRIDGES program!
