Connecting You to Trusted Support. Navigating the complexities of aging, caregiving, and health can be challenging, but you do not have to do it alone. Summit County Public Health partners with trusted local, state, and national organizations to ensure older adults and their families have access to the support they need. Use the links below to connect directly with our community partner agencies.

Local & State Aging Resources

  • Direction Home Akron Canton Area Agency on Aging: Providing long-term care choices, resource navigation, and support services for older adults and individuals with disabilities across Portage, Stark, Summit, and Wayne counties.
     
  • Alzheimer's Association: Greater East Ohio Chapter:Offering a 24/7 helpline, local support groups, care consultations, and educational programs for individuals living with Alzheimer's disease and their families.
     
  • Golden Buckeye Center for Dementia Caregiving: A statewide resource dedicated to supporting families and caregivers of Ohioans living with Alzheimer's disease and other dementias through expert training, education, and connection. 
     
  • Adult Protective Services (APS):If you suspect an adult age 60 or older is experiencing abuse, neglect, or exploitation, please report it. You do not need proof to make a report, and your information is kept confidential by law.

  • Community Resource Navigation provided by Senior and Adult Services @ SCPH: If you are an older adult looking for ways to stay independent, healthy, and safe in your own home, our team can help connect you to the right local agencies. We provide guidance on: 
    • Fall prevention and home safety
    • Nutrition and meal delivery programs
    • Social and community engagement opportunities
       
  • Adult Guardianship Services of Summit County: Providing compassionate guardianship and advocacy for vulnerable older adults who are unable to safely manage their own personal, medical, or financial affairs.
