Ordering Ohio Birth and Death Certificates Made Quick and Easy
Summit County Public Health now offers certified birth and death certificate copies for all Ohio counties for any person born or who died in the State of Ohio. This covers births from December 20, 1908, to the present and deaths from January 1, 1971, to the present. SCPH does not have birth or death certificate records for other states or countries. Uncertified birth, death, or fetal death certificates are not available per Ohio law.
You can also obtain certified birth and death certificate copies from any local health department in Ohio including the Ohio Department of Health. Fees for certified birth and death certificate copies are set by each local health department and range from $21.50 to $27.00 per record. The cost to purchase a certified birth or death certificate at Summit County Public Health is $22.00 per copy. Cash, checks, money orders, major debit/credit cards are all accepted (Note: Cardless/digital wallet payment is not accepted i.e., Apple Pay, Tap Pay, etc.)
Visit our office at Fairway Center for Same-Day Service today. No appointment or ID is required. Any person who knows the name listed on the record and the event date may purchase the birth or death certificate. However, if the purchase is for a death certificate issued within five years of the deceased person’s death that requires the deceased person’s SSN to appear on the record; in that case, proof of relationship is required. Total processing time takes less than 10 minutes. We also provide accessible support services with curbside order delivery upon request.
