Order by Mail

Certified birth, death and fetal death certificate copies for every person born or who died in the State of Ohio from December 1908 – present are available for purchase by mail.  SCPH does not have birth or death certificate records for other states or countries.  

Download, complete and mail the Application for Certified Copies form or send a typed or handwritten records request that includes:

  1. The name of the person requesting the certificate
  2. The street address, city, state and zip code of the person requesting the certificate  *Include the shipping address if different than the person requesting the certificate
  3. The email address and phone number of the person requesting the certificate
  4. The name on the birth or death certificate when the person was born or died
  5. The record type:  Birth – Death – Fetal Death  
  6. Reason for ordering the birth or death certificate (i.e., Employment, Estate Settlement, Passport, Adoption, Housing, Driver’s License, etc.)

Include the fee amount of $22.00 for each certificate requested.  Uncertified birth, death, or fetal death certificates are not available.  Make personal/cashier’s checks, or money orders Payable to SCPH.  Please DO NOT SEND CASH.  Mail order requests are also available by phone.  Call our office at 330-812-3845 to place an order (A major credit or debit card is required.)

Mail order requests are processed the same day the order is received. Completed mail order requests will be shipped by USPS First-Class Regular Mail.   Please allow 7 – 10 business days to receive your processed request. 

Note:  SCPH has no way of tracking delivery of USPS First-Class Regular Mail orders and is not responsible for replacing lost, delayed, or undelivered mail requests.  For guaranteed mail order delivery as soon as the next business day to any state within the United States, call our office at 330-812-3845 to place an order (A major credit or debit card is required.  Additional processing and shipping fees apply.)
