Nurse Family Partnership

Nurse-Family Partnership (NFP) is a free national evidenced-based community health program for first time moms. Each mother served is partnered with a registered nurse and will receive home visits starting earlier in pregnancy through the baby's second birthday. The specially trained personal nurse will assist first time moms and families become more knowledgeable about pregnancy, labor and delivery, and life with baby. The nurse will also provide knowledge and support around child growth and development and link families to community services and resources. Nurse-Family Partnership nurses help moms to be the best mom they can be.

ELIGIBILITY

  • Must be a first time mom
  • Must enroll by the 28th week of pregnancy
  • Must meet income requirements
